No Travel Worker will be considered here Title: OSH Registered Nurse (RN) Duration: 3 Months( with possibility to extend or convert) Shift: Monday–Friday | 8:00 AM – 5:00 PM Location: Baton Rouge, LA (Primary: Delmont Village) Job Description: ?/tThe OSH Registered Nurse (Center Nurse) is responsible for delivering patient-centered, coordinated care in a clinic-based value care environment. This role focuses on chronic disease education (diabetes and cardiovascular disease), telephonic triage, medication management, and care plan coordination. The RN will collaborate with interdisciplinary teams to improve patient outcomes, reduce ED/hospital utilization, and enhance patient engagement.The MA coordinates scheduling, pre-authorizations, record collection, and testing completion prior to appointments or surgical procedures. The position also serves as a liaison between providers and patients, ensuring clear communication regarding care timelines and treatment planning. ?/tRoles and Responsibilities: ?/tProvide competent, safe, and evidence-based nursing care in a clinic setting. ?/tDeliver comprehensive patient education, particularly for diabetes and cardiovascular disease management. ?/tConduct telephonic triage and symptom management using standardized protocols. ?/tPerform medication reconciliation, prescription refills, and prior authorization processing. ?/tAssess patients both face-to-face and remotely with strong clinical judgment. ?/tCollaborate with interdisciplinary care teams to implement and monitor care plans across multiple provider panels. ?/tSupport preventative care initiatives and hospital diversion strategies. ?/tRespond to incoming patient calls in a timely and professional manner. ?/tPerform point-of-care testing, specimen collection, and phlebotomy as needed. ?/tDocument accurately in EMR systems and maintain strict patient confidentiality. ?/tParticipate in care team meetings and clinic operational discussions. ?/tDelegate appropriate tasks to clinical team members to optimize workflow. ?/tTravel up to 50% between Baton Rouge, New Orleans, and Lafayette as required. Required skills/Experience: ?/tActive Louisiana RN License or Compact State License (in good standing). ?/t2–5 years of RN experience (3–5 years preferred). ?/tBLS Certification. ?/tPrior clinic/outpatient experience preferred. ?/tStrong EMR navigation and computer skills. ?/tAbility to independently assess patients and work autonomously. ?/tExcellent communication and patient education skills.
