For many of our team members, this is more than a career- it's a calling.Licensed Practical Nurse Position SummaryParticipate in directed care delivery including functional and focused nursing tasks as assigned by the RNProvide ADL and other support as directedReport observations and complete all required documentation in a timely mannerFunction as part of a nursing team supervised by a Registered NursePrincipal AccountabilitiesSubmit appropriate documentation and paperwork to facilities at the completion of patient visitsSubmit all required documentation in a timely mannerAttend and participate in staff meetings, educational programs and community events as requestedCommunicate identified needs and potential solutions to supervisorComplete documentation at the bedsideAdhere to standards of nursing practice and maintain knowledge of current clinical practice for terminally ill patientsParticipate in quality performance improvement projects and clinical record reviews as directedCommunicate effectively with RNCM, DCS and physicians (as applicable)Provide nursing services as directed by the RNAdminister medications, treatments, etc. as directedGather clinical data and provide to RN via documentation and communication in order to support comprehensive assessment and Plan of Care managementAttend IDT meetings when ableProvide education and reinforcement of teaching related to:Care of the patientDisease processSymptom managementMedications, including expected effect, side effects, and when to call hospiceWound careThe dying processEducation and ExperienceHigh school diploma or general education degreeCurrent nursing license for the specified state and minimum two years full-time experience as a Licensed Practical/Vocational Nurse; or equivalent combination of education and experienceExperience in skilled nursing, preferably in hospice, home health care, oncology or emergency carePerformance within scope of practice per state Nurse Practice Act is requiredStrong written and verbal communication skillsStrong mathematical and problem-solving skillsKnowledge of Microsoft Office programsExperience with EMR (electronic medical records) preferredSkills and AbilitiesUnderstanding and acceptance of hospice philosophyHigh personal integrity including maintenance of confidential information and all patient recordsStrong organizational and time management skillsMust be able to work a flexible schedule including nights and weekendsMust be able to travelAbility to work with patients who are terminally ill and their family membersAbility to understand, read, write, and speak EnglishPhysical demands include:Ability to use hands to finger, handle or feelAbility to talk and hearAbility to stand, sit and reach with hands and armsAbility to walk, climb, balance, stoop, kneel, crouch or crawlAbility to taste and smellAbility to lift and/or move more than 50 poundsSpecific vision abilities including close visionThe physical demands described here are representative of those that must be met by a colleague to successfully perform the essential functions of this job.
