As a result, this gives teammates the opportunity to excel in an award-winning environment, enabling them to thrive professionally and personally.The CARE COORDINATOR will 'coordinate' with providers and Integrated Care Nurses to deliver ongoing support and communication to End Stage Renal Disease (ESRD) patients. The Care Coordinator acts as a single point of contact to 'coordinate' resources for DaVita IKC patients.*LOCATION: HYBRID - in clinic, with some remote days. - MUST be located in the LAFAYETTE / LAKE CHARLES, LOUSIANA area**mileage reimbursementEssential Duties and Responsibilities:

• Performs outbound calls and responds to inbound calls from members, providers and Village resources
• Complete non-clinical pathways & screenings, under guidance of Supervisor; escalates to Integrated Care Nurse, as needed
• Provide support, guidance and coordination of care for ESRD patients
• Help identify eligible resources for patients and members
• Assist with the program referral process
• Collaborate with Integrated Care Nurses, dialysis clinics, health plans and other stakeholders to help patients identify & enroll in eligible programs
• Support in providing detailed, patient-specific education about these programs
• Coordinate provider appointments, on behalf of DaVita IKC members
• Obtain patient PAM & PHQ-9 forms, ensuring completion
• Connect with insurance companies, when necessary, regarding patient care and benefits
• Responsible for accurate documentation of patient-related information
• Accountable for outstanding customer service to all external and internal patients and teammatesQualifications:
  • Intermediate proficiency in MS Word, Excel, PowerPoint, and Outlook
  • At least 2 years of experience in a professional office environment
  • High School Diploma
  • Patient Care Technician or Administrative Assistant experience in dialysis field preferred
