Job Summary

Curana Health is a leading provider of value-based primary care services for the senior living industry. We are seeking a skilled Nurse Practitioner to join our team and provide high-quality care to our residents.

Job Responsibilities

The successful candidate will be responsible for administering direct patient care services within the Post-Acute Care setting. This will include conducting clinical interventional and episodic direct care visits, evaluating and treating chronic and co-morbid illnesses, and coordinating care with collaborating physicians. Additionally, the Nurse Practitioner will be responsible for conducting Annual Wellness Visits and Annual History and Physicals as requested by facilities.

Requirements

To be successful in this role, the candidate must have a valid nurse practitioner license in the state of operations and current board certification in their field. A Master's Degree in Nursing or Physician Assistant Studies is also required. Strong leadership, interpersonal, and problem-solving skills are preferred. The ideal candidate will possess excellent written and oral communication skills and be able to function within a team environment.
