Center-AP-Physician-0011 State LA Location 1214 Coolidge St.
Lafayette Louisiana 70503 Requirements Active State Medical
License, CDS, & DEA (with LA address) required to start
credentialing process, Board Certified or Board Eligible in ABIM.
*Must be residency trained in Specialty Certified in (only
recognize board certification through American Board of Medical
Specialties ABMS)*, ACLS, BLS, CME past 2 years, case logs past 2
years, copies of all education & training diplomas, SS Card,
Driver's license, color photo, Flu, malpractice coverage since
Residency and including Residency. Will accept H-1B and J-1 VISA
candidates. PPD no longer required.Average Daily Census: 22011 day
rounders, 1 nocturnist2 day NPPA rounders, 1 NPPA nocturnistShift
Time for Rounding: 7a-7pProcedures not requiredFull Specialty
