community partners to gain an understanding of their needs and
assists with the creation of plans and projects. Oversees and
implements the Population Health Plan as a coach and liaison to
serve the communities surrounding the Community Health Centers.
Represents Ochsner Health when developing and fostering
relationships with members of the community (including community
coalitions, neighborhood organizations, schools, libraries, parks,
social service agencies, faith leaders, residents, Federally
Qualified Health Centers, health homes, local businesses, specialty
providers of care, local hospitals and clinics) to ensure the
Community Health Center meets the needs of the community it serves.

Business, Public Relations, Public Health, or relevant field OR 4
additional years of relevant work experience will be accepted in
lieu of educational requirements Preferred - Master's Degree in
Public Health, Healthcare Administration, Business Administration,
or related field of study Work Experience Required - 3 years of
experience in community relations, business development, marketing,
public health, or other relevant healthcare industry experience
Preferred - Grant writing experience Knowledge Skills and Abilities
(KSAs)
- Must have computer skills and dexterity required for data entry
and retrieval of information.
- Strong interpersonal skills.
- Strong written and verbal communication skills to effectively
communicate project plans, and uncover community needs with
stakeholders including community leaders, patients, caregivers, and
internal stakeholders.
- Ability to balance the needs of the Community Health Centers
and the community served through an inclusive and collaborative
approach.
- Must be able to develop and deliver presentation materials as
they convene and facilitate meetings and manage Community Health
Center advisory groups.
- Strong interpersonal skills to manage and foster relationships
with a variety of stakeholders for the purposes of advancing the
mission of the Community Health Centers.
- Ability to work evenings, nights and weekends as needed. Job
Duties
- Identifies opportunities within assigned regions and develops
pipelines to reach key populations of individuals that the
Community Health Center serves.
- Convenes, facilitates, and manages Community Health Center
advisory groups and aligns the efforts of the advisory groups
established in various regions to support community-level
collective impact of initiatives to improve community health.
- Actively engages internal and external stakeholders to provide
health education and promotes the initiatives of the Community
Health Center.
- Ensures all efforts in support of Community Health Center are
aligned with the Department of Population Health, as well as key
internal and external stakeholders.
- Collaborates with business development to grow partnering
relationships with local community businesses to support the
Community Health Center mission.
- Effectively coordinates efforts with physician liaison members
to ensure alignment of clinical and community strategies.
- Collaborates with Government Relations to open doors and expand
influence in assigned markets with government and civic
groups.
- Designs and maintains databases, spreadsheets, and listservs to
report and track community participation and results of various
initiatives in the communities served.

exerted is negligible. Normal routine involves no exposure to
blood, body fluid or tissue, but exposure or potential for exposure
may occur. The incumbent works with patients who have known or
suspected communicable diseases and may enter isolation rooms.
There may be an occupational risk for exposure to communicable
diseases. Because the incumbent works within a healthcare setting,
there may be occupational risk for exposure to hazardous
medications or hazardous waste within the environment through
receipt, transport, storage, preparation, dispensing,
administration, cleaning and/or disposal of contaminated waste. The
risk level of exposure may increase depending on the essential job
